You can create a new instance of a document by hitting the "Create new" button:

This button is contextual, and will help you add a document to the current list. For instance:
- If you're on the "Work" tab, it will ask you to choose which document type to use.
- If you're in a list filtered by a specific custom type, you won't have to choose: this button will create a new instance of this type.
- If you're in a list filtered by specific tags, your new empty document will be immediately marked with those tags.
You can directly create a document inside a release by selecting the release and hitting the "Create new" button.
